The doctors at Warrandyte Road Clinic are proud to have cared for the Ringwood community for many years. They look after a large number of senior patients, many of whom they have supported through various stages of life.

Annual 75+ Health Assessment

For our regular patients aged 75 and over, we recommend an annual comprehensive health assessment. This is a thorough, in-depth consultation funded by Medicare that allows us to look at your health holistically, rather than just treating a specific illness.

During this assessment, we review:

  • Physical Health: Blood pressure, pulse, weight, and general mobility.
  • Daily Living: Your ability to manage tasks at home, such as dressing, showering, and cooking.
  • Medication: A complete review of all medications to ensure they are still necessary and safe.
  • Sensory Health: Basic checks of your vision and hearing.
  • Cognitive Function: Screening for memory issues or early signs of dementia.
  • Safety: Assessing fall risks in your home environment.
  • Immunisation: Ensuring you are up to date with flu, shingles, and pneumococcal vaccinations.

Chronic Disease Management

Many seniors live with one or more chronic conditions, such as arthritis, diabetes, heart disease, or osteoporosis. We can put together a GP Chronic Condition Management Plans (GPCCMP) to coordinate your care.

These plans allow you to access subsidised visits to allied health professionals, such as:

  • Podiatrists for foot care.
  • Physiotherapists for mobility and strength.
  • Dietitians for nutrition and weight management.
  • Diabetes Educators for blood sugar management.

Medication Reviews

Taking multiple medications (polypharmacy) can be confusing and increases the risk of side effects. We can arrange a Home Medicines Review (HMR).

In this process, a pharmacist visits you at home to go through your medicine cabinet, ensuring you understand your dosages and checking for any interactions between your prescription drugs and over-the-counter supplements. They then report back to your GP to streamline your script.

Cognitive Health & Dementia Support

We understand that memory loss is a significant concern for many seniors and their families. Our GPs provide a supportive environment to discuss cognitive changes. We can perform initial memory screenings and refer you to geriatricians or memory clinics if a more detailed diagnosis is required.

Advance Care Planning

We encourage all our senior patients to discuss their future health wishes. We can assist you in creating an Advance Care Directive, ensuring your family and doctors understand your preferences regarding medical treatment should you be unable to communicate them yourself.
