We offer free, annual, comprehensive health assessments to all patients aged 75 years and over.

This age represents the time at which people are most likely to experience the greatest levels of disability and ill health.

What is a health assessment?

Annual health assessments provide a comprehensive review of your health status, allowing for early detection of potential issues and personalized care planning. It’s a way to inform your GP of risks and hazards which may require further health management. In addition to assessing a person’s health status, a health assessment is also used to identify factors that influence your physical function, psychological function and social function.

What are the benefits of a health check?

  • You can raise any concerns that you may not usually have time to discuss with your doctor in a standard consultation.

What is involved?

  1. registered nurse will perform a comprehensive interview, usually at the clinic, or sometimes in your home. We will check your blood pressure, pulse, height, weight and vision, and perform a urinalysis. Part of the assessment also involves discussing your home environment to assess your living arrangements to see if they are safe and if there is any additional help you may need. All of this takes about 45 minutes.
  2. You subsequently attend your usual doctor for a medical checkup and completion of the assessment.

Does it cost anything?

The annual health assessment is free. This includes both the consultation with the nurse and and the follow-up appointment with your doctor.
